When A Family May Need Help
- When geographically distanced from parents or relatives
- If you are unfamiliar with medical conditions and terminology.
- Lives alone
- Questionable support system
- Recent trauma or sudden change in functioning
- Client is caregiver for family member
- Questionable abuse or neglect
- Multiple medical problems
- Environmental problems
- If a Medicaid expert is needed to process application and act as representative
- Need for community resources and linkage
- Need for long term planning/placement
- Non-compliance with treatment plan
- Depression or anxiety
- Recurrent hospitalizations
- Terminal illness
- Substance abuse
