Referral Guidelines for Attorneys and Financial Practitioners
- When the client is mentally ill and would need the assistance of a Human Services professional to provide care coordination.
- When the attorney or financial practitioner needs consultation regarding the care options and resources available.
- When counseling services are needed to assist patient transition into nursing homes.
- When a guardian or conservator is considered or appointed.
- When there is a history of recurrent hospitalizations.
- When a person needs to be connected with public services.
- When there is a need for help with financial management.
- When there may be a need for multiple services-i.e. home health care, companions, rehabilitation, daycare, and visiting nurses.
- When multiple services are already being provided and need coordination.
- When personal advocacy is needed for quality of care in a nursing home.
- When a change in level of care is considered-i.e. placement
- When there is a conflictual family situation and an objective assessment of the client's needs is required
